For Junior High School

• Original Grade 10 report card with the applicant’s eligibility for admission to senior high school duly signed by the school principal
• Original Certificate of Good Moral Character (dated not earlier than February of the completion year) from junior high school (with school seal)
• Original birth certificate printed on Philippine Statistics Authority (PSA) paper

For Junior High School completers who did not enroll the following school year in Senior High School

• Original and photocopy of Grade 10 report card
• Photocopy of certification from the junior high school principal stating that the applicant’s Form 137-A is still in the school file and has not been forwarded to another school
• Original Certificate of Good Moral Character (dated not earlier than February of the completion year) from junior high school (with school seal)
• Original birth certificate printed on PSA paper

For foreign enrollees who completed junior high school abroad except from Philippine schools under DepEd

• Scholastic records, duly authenticated by the Philippine Foreign Service Post located in the student applicant’s country of origin or legal residence. If the country or territory of origin is already a member of the Apostille Convention, authentication of the Philippine Foreign Service is no longer necessary.
• Certificate of completion of Grade 10 (junior high school / Grade 10, or its equivalent)
• Photocopy of applicant’s passport bio-page, latest admission, and authorization to stay.
• Original Certificate of Good Moral Character (dated not earlier than February of the completion year) from junior high school (with school seal)

The above-mentioned admission credentials should be submitted together with the following downloadable Mapúa forms in one (1) long brown envelope. The forms should be printed on long bond paper and should be completely filled out by both the student and the parents/guardian.

For foreign nationals, please report to the Office of the Registrar, Window 5, to present your passport and ACR-ICARD so we can assist you in the processing of your visa.

For Mapúa UOX enrollees, submission of documents is either onsite through an authorized representative or via courier.

Note: We will inform you through email confirming that we have received the above-mentioned admission credentials and the Mapúa forms. The university is not liable for any losses once you submit it via courier.

If there are discrepancies, you will also be notified through e-mail. It is important that you comply as soon as possible so that you will be cleared for enrollment and your school IDs will be released to you upon their availability.
