Admission Guidelines

APPLICATION REQUIREMENTS:

  1. Completely filled-out Application form.
  2. Original or certified true copy and photocopy of Grade 9 report card
  3. Proof of candidacy for completion (any of the following)
    • Photocopy of report card for the current school year
    • Photocopy of Certificate of Good Moral Character
    • Photocopy of Certificate of Enrolment
    • Photocopy of current school ID with school year & year/grade level
  4. 2 pcs. ID picture (plain background), size 1½” x 1½”
  5. Application fee (if applicable)
  6. Additional Requirements:
    • For junior high school completers who did not enrol the following academic year in Senior High School (completion year is 2016 and prior):
      • Original and photocopy of junior high school (Grade 10) report card
      • Photocopy of certification from the high school principal stating that the applicant’s Form 137-A is still in the school file and has not been forwarded to another school
      • Additional requirements for non-Filipino applicants from Philippine schools:
        • Alien Certificate of Registration (ACR) or Special Study Permit (SSP)
        • Original and photocopy of Passport
    • For applicants who attended Non-Formal Education Accreditation and Equivalency or completers of Alternative Learning System Accreditation and Equivalency:
      • Certificate of Rating (with passing marks in all subjects)
    • For foreign applicants who completed their junior high school abroad except from Philippine schools and under DepEd:
      • Scholastic records or Transcript of Records duly authenticated by the Philippine Embassy in the applicant’s country of origin
      • Certificate of Completion in Grade 10 (junior high school or equivalent)
      • Photocopy of applicant’s passport bio-page, latest admission and authorized stay

IMPORTANT REMINDERS :

  1. Applicants with at least 85% General Weighted Average (GWA) and final grades in Science, Mathematics, and English in Grade 9 are exempted from taking the Mapúa Scholastic Aptitude Examination (MSAE).
  2. Applicants who attended Non-Formal Education Accreditation and Equivalency or completers of Alternative Learning System Accreditation and Equivalency shall be required to take the MSAE.
  3. All applicants who are interested to study senior high school in Mapúa, regardless if they are exempted or not in taking the exam, shall provide all admission requirements.
  4. Applicants exempted from taking the MSAE may confirm their slots by paying the non-refundable reservation fee of Php 5,000 at the Mapúa Treasurer’s Office starting August 2016.
  5. The application is considered official once a receipt has been issued (for MSAE takers):
    • MIT’s official receipt (for those who applied in-campus) or
    • MIT’s provisional receipt (for those who applied at any accredited Testing Center)
  6. Examinees should present their examination permits together with the receipts on the day of the examination.

ENROLLMENT REQUIREMENTS:

  1. Mapúa Examination Permit or Examination Result Letter or Notice of Results
  2. Original Grade 10 report card with the applicant's eligibility for admission to Senior High School duly signed by the school principal
  3. Original certificate of good moral character (dated February 2017 onwards) from the high school (with school seal)
  4. Original birth certificate printed on National Statistics Office (NSO) paper
  5. Fully accomplished SHS Student Data Sheet / SHS Enrolment Form
  6. 1 pc. ID picture (plain background), size 1½” x 1½”
  7. One (1) long brown envelope to contain all of the above
  8. Additional/Other Requirements:
    • For junior high school completers of previous years (completion year is 2016 and prior) who did not enroll in any school after completion:
      • Certification from the high school principal stating that the Form 137-A is still in the school file, and has not been forwarded to another school
    • For students who attended Non-Formal Education Accreditation and Equivalency or completers of Alternative Learning System Accreditation and Equivalency:
      • Certificate of Rating with passing marks in all subjects
      • Photocopy of diploma
    • For Filipino students coming from schools abroad (except for Filipino schools under DepEd):
      • Authenticated Transcript of Records from Junior High School (in lieu of number 2)
      • Certificate of Completion in Grade 10 (junior high school or its equivalent)
      • Certificate of Good Moral Character or Recommendation from the School Guidance Counselor or Principal (in lieu of number 3)
    • For foreign nationals:
      • Current junior high school completer (completion year is 2017) from high schools in the Philippines or high schools abroad under DepEd:
        • Alien Certificate of Registration (ACR) or Special Study Permit (SSP)
        • Original and photocopy of Passport
        • Photocopy of birth certificate from the country of origin
